ii) a copy of the applicant’s college transcript (applicants may later be asked to supply an official transcript), and 

iii) letters from two mathematicians familiar with the applicant’s work. 

Optional forms for the use of faculty references are supplied. Recommendation 
letters may be returned to applicants in signed, sealed envelopes and mailed 
with the other parts of the application, or they may be submitted directly 
by the recommender (directions for doing this are listed on the bottom of 
the Optional form).
